REQUIREMENTS FOR:
A) RE-ISSUE OF A FRESH PASSPORT
1. Application Form, filled online (https://embassy.passportindia.gov.in/ )
2. Photocopy of Passport, self-attested
3. Photocopy of Residence Permit / Work Permit, self-attested
4. Two recent colour Photographs, Size: 5cm * 5cm with 60 - 70% of face. (One pasted on application)

B) LOST/DAMGED PASSPORT PASSPORTS
1. Application Form, filled online (https://embassy.passportindia.gov.in/ )
2. Photocopy of lost Passport, self-attested
3. Photocopy of Residence Permit / Work Permit, self-attested
4. FIR from Greek Police, for lost passport
5. Four recent colour Photographs, Size: 5cm * 5cm with 60 - 70% of face. (One pasted on application)
6. Affidavit of lost passport (may be obtained from Embassy)
7. An affidavit that the applicant remains an Indian citizen (click here to download)
C) CHILD'S PASSPORT AND BIRTH REGISTRATION CERTIFICATE
1. Application Form, filled online (https://embassy.passportindia.gov.in/)
2. Photocopy of Passport, attested by parent
3. Photocopy of Residence Permit, attested by parent
4. Translated copy of birth certificate from the Ministry of Foreign Affairs along with photocopy of the original birth certificate in Greek (for infants)
5. Marriage certificate duly attested by Ministry of External Affairs (for Infants)
6. Photocopies of parents passport along with original and Residence Permit / Work Permit, self-attested
7. Two recent colour Photographs, Size: 5cm * 5cm with 60 - 70% of face. (One pasted on application)
8. An affidavit of parent that the child remains an Indian citizen (click here to download)

5. ATTESTATION OF DOCUMENTS: India is one of the signatories to The Hague Apostille Convention and the new scheme was implemented in India on 29.8.2007. The purpose of the Convention is to reduce the two stage authentication of public documents to one single stage. If a public document is apostilled by the country of origin, it does not require re-authentication by the authorities of the country in whose territory it has to be produced, provided both the countries are party to the Apostille Convention. With India's accession to the Convention, re-authentication of documents in Greece is not necessary as Greece is also a party to the Apostille Convention.
